Our records indicate that you are a medical provider that has practice patterns and/or return to work outcomes that meet our expectations. Accordingly, we would like to include you on our medical provider referral list that we give to injured workers who are seking referrals. We do not ask our referral providers to take any additional discounts beyond the BWC fee schedule. Additionally, we do not require a signed contract for inclusion on our referral paneland you can opt out at any time by contacting our office.

Dr. Black correctly and efficiently diagnosed my industrial injury and hypertension. He also provided me with expert advice, referrals to specialists and decompression treatments. He also helped me work with the BWC in resolving my claim. His efforts eliminated the need for cervical surgery.
